Ageless by Reva (“Ageless by Reva,” “we,” “us” or “our”) is an injectables studio in Las Vegas, Nevada, led by Reva Frey, RN. This Privacy Policy explains how we collect, use and protect information when you visit agelessbyreva.com (the “Site”), contact us, or book a consultation online. By using the Site, you agree to the practices described here.

Information we collect

Information you give us. When you fill out our contact form, book a consultation, call, text or email us, we may collect:

• Your name, email address and phone number
• The treatment(s) you are interested in and the content of your message
• Appointment details, such as your preferred date and time

Information collected automatically. Like most websites, our hosting, security and site tools may automatically record technical information such as your IP address, browser type, device type, pages viewed, referring page and the date and time of your visit. Some of this is collected through cookies and similar technologies (see “Cookies” below).

Please do not send medical information through the Site. Our contact form and online booking are not designed for sharing detailed medical history, photos or other health information. We will gather the health information we need privately during your consultation.

Health information and patient records

Health information you provide during your consultation and treatment — such as your medical history, consent forms, treatment notes and before-and-after photos — is part of your patient record. It is kept confidential and handled in accordance with applicable federal and Nevada privacy laws. This Privacy Policy covers information collected through the Site; any separate patient privacy notice you receive at our studio governs your patient record.

• To schedule, confirm, reschedule or remind you about appointments
• To provide our services and follow up after treatment
• To keep the Site secure, prevent spam and fraud, and fix technical problems

How we share your information

We do not sell or rent your personal information. We share information only in these limited situations:

• Service providers who help us run our business and the Site — for example our website host, security tools, email provider and online scheduling service (Calendly). They may use your information only to provide services to us.
• Legal reasons — when required by law, subpoena or court order, or to protect the safety, rights or property of our clients, our business or others.
• Business transfers — if our business is sold or transferred, information may be transferred as part of that transaction, subject to this Policy.
• With your consent — for any other purpose you approve.

Online booking and third-party services

When you book a consultation, you may be taken to Calendly, a third-party scheduling service. Information you enter there is also subject to Calendly’s own privacy policy. Our Site also contains embedded Google Maps and links to Instagram, Facebook, Yelp and Google. Those services may collect information under their own privacy policies, and we are not responsible for their practices.

Cookies

Cookies are small files stored on your device. We use cookies that are necessary for the Site to work and stay secure, and third-party features on the Site (such as maps and booking) may set their own cookies. You can block or delete cookies in your browser settings, but some parts of the Site may not work properly if you do. Our Site does not currently respond to “Do Not Track” browser signals.

Text messages and calls

If you give us your phone number, we may call or text you about your inquiry or appointments. Message and data rates may apply. You can reply STOP to any text message to opt out of further texts, or ask us not to contact you by phone.

How we protect your information

We use reasonable administrative, technical and physical safeguards to protect your information, including secure (HTTPS) connections and website security monitoring. No website or method of transmission is 100% secure, however, so we cannot guarantee absolute security.

How long we keep information

We keep inquiry and booking information only as long as needed for the purposes described above. Patient records are kept for the period required by Nevada law and professional standards.

Your choices and rights

• You can ask to see, correct or delete the personal information you have given us through the Site, subject to legal and record-keeping requirements.
• You can unsubscribe from marketing emails using the link in the email or by contacting us.
• Nevada residents: we do not sell covered information as defined in Nevada law (NRS 603A). You may still send an opt-out request to info@agelessbyreva.com and we will honor it.

Children’s privacy

The Site is intended for adults. We do not knowingly collect personal information online from children under 13. If you believe a child has provided us information, please contact us and we will delete it.

Changes to this policy

We may update this Privacy Policy from time to time. The updated version will be posted on this page with a new effective date. Your continued use of the Site after changes are posted means you accept the updated Policy.
